Submit an addition, a deletion or a change to the Faculty, Staff and Postdoctoral Fellow Directory
Please enter information on the SPH Faculty or Staff member that you would like added to the directory, deleted from the directory, or changed in the directory. An email will get sent to the appropriate staff member in your department, who will complete the request. Required fields are denoted with an asterisk(*).
If you need to update your MCommunity record, please use the Personal Data Change Request eForm.
To keep the directory consistent, please observe the following conventions:
- Use roman numerals for SPH I and SPH II
- Street addresses for SPH I and SPH II are omitted
- All room numbers in SPH II start with M (e.g., M2079 SPH II)
- Please do not use commas in addresses
- Addresses that were formally designated as "SPH Tower" or "SPH Crossroads" are now designated as "SPH I"
- Phone numbers do not include the area code, but do include the full exchange
Departmental directory updates can be made directly by authorized personnel. Requests for changes to Faculty Profiles should be sent to the appropriate departmental Web Content Manager. If you have a large batch of updates to make, share a Google Sheet with chrisdia, or send an excel spreadsheet to [email protected]. Be sure that each row contains a valid uniqname.
