Complete the following form to revoke permission for the University of Michigan School of Public Health, or anyone authorized by the school, to record your photographs or other images or likenesses in the form of audio, video, or any other medium, or to use, reproduce, modify, distribute, or publicly exhibit such recordings. 

Please be aware, the School of Public Health does not seek specific permission when video recording or photographing in a public space where people do not have a reasonable expectation of privacy--for example, in public lobbies, lounges, hallways, sidewalks, cafes and restaurants, parks, a stadium, and similar locations. If you would like to ensure that you are not recorded, please alert the photographer or videographer.
