As a reminder, this form is not a reporting mechanism.  To report a concern, please use the existing reporting mechanisms and processes from the following University of Michigan offices:  

• Complete a Discrimination, Discriminatory Harassment and Sexual Misconduct Reporting Form with the Office of Institutional Equity 
• File a Student Support Request Form with the Dean of Students Office
• Report a crime or concern with the Division of Public Safety and Security
• Review Sexual and Gender-Based Misconduct Reporting and Resources for a safe and respectful community

The following are confidential support resources available to our community:

• Sexual Assault Prevention and Awareness Center
• Ombuds: The Office of the Ombuds for Students,  Office of the Staff Ombuds or Office of the Faculty Ombuds
• For Students: UM Counseling and Psychological Services (CAPS) and SPH embedded CAPS counselor
• For Faculty and Staff: Faculty and Staff Counseling and Consultation Office 
• Career & Education Counseling at CEW+
• SafeHouse Center, local nonprofit for survivors of sexual assault and domestic violence
• Praesidium: External provider available to those impacted by Martin Philbert or Robert Anderson. Call Praesidium at 888-961-9273 to learn more about how to access confidential counseling resources
