Michigan Public Health Feedback Form
Purpose
This form is an anonymous form created for the School of Public Health Community to provide feedback and ideas. Information provided on this form will be used to help determine training, and influence future planning to improve our school climate. The feedback also will help create the action items as we continue to build a community of inclusion where all community members feel safe, valued and able to contribute.
This form is not a reporting mechanism. To report a concern, please use the existing reporting mechanisms and processes from the following University of Michigan offices:
- Complete a Discrimination, Discriminatory Harassment and Sexual Misconduct Reporting Form with the University of Michigan’s Equity, Civil Rights and Title IX Office
- File a Student Support Request Form with the Dean of Students Office
- Report a crime or concern with the Division of Public Safety and Security
- Review Sexual and Gender-Based Misconduct Reporting and Resources for a safe and respectful community
As a reminder, the following are confidential support resources available to our community.
- Sexual Assault Prevention and Awareness Center
- Ombuds: The Office of the Ombuds for Students, Office of the Staff Ombuds or Office of the Faculty Ombuds
- For Students: UM Counseling and Psychological Services (CAPS) and SPH embedded CAPS counselor
- For Faculty and Staff: Faculty and Staff Counseling and Consultation Office
- Career & Education Counseling at CEW+
- SafeHouse Center, local nonprofit for survivors of sexual assault and domestic violence
- Praesidium: External provider available to those impacted by Martin Philbert or Robert Anderson. Call Praesidium at 888-961-9273 to learn more about how to access confidential counseling resources
Responses to this form will be sent to Dr. Whitney Peoples, SPH Diversity, Equity and Inclusion Director, and Andrea Lubetkin, SPH HR Director, who will summarize responses for school leadership to inform climate improvement actions. While annual summaries will be retained, individual submissions will not.
