If you are a permanent faculty or staff member (having an ongoing and renewable appointment expected to last one year or more from the date of request), EHS will provide one pair per eligible employee. To request a voucher beforehand, please visit ehs.umich.edu and click on the "Safety Shoe/Eyeglass Request Form" at the bottom of the home screen under "Worker Safety". If a voucher is not requested prior to the date and time you wish to come, you will have the opportunity to obtain a voucher onsite.
